There are over 100 million people in the United States suffering from chronic pain every year, and the cost of this pain is over $500 billion every year. Multiple misperceptions exist in pain management. Here are a few of them described. </p>
<p>1. Narcotics are always bad. Each year, there are over 15,000 overdose fatalities from narcotics and there is plenty of bad press on a weekly basis regarding narcotic pill mill medical practices. So a significant amount of the general population looks at narcotics as always being a bad thing.</p>
<p>However, opiate medications do in fact have a legitimate place in pain management. For instance, a patient who has cancer may live his or her last few months in horrific pain if they do not have narcotics to assist in the relief that they deserve. </p>
<p>In individuals who do not have an indication for surgery and are dealing with chronic pain, opiates as a pain management treatment may allow individuals to maintain employment and to enjoy functional lives and to continue playing with their kids. </p>
<p>So narcotic medications do actually have a place in treating acute and some types of chronic pain in patients. </p>
<p>2. MRI will always give you the diagnosis for a person&#8217;s back pain. Even with the best trained medical doctors, currently it is only possible 50% of the time to give patients a real definitive diagnosis for what is causing their back pain.</p>
<p>This includes individuals who have had a comprehensive workup of the  musculoskeletal system including x-rays, history and physical, and even an MRI. So an MRI is not always the most trustworthy piece of the puzzle. </p>
<p>3. Back surgery fixes most back pain. A significant amount of time there is not really a true cure for a person&#8217;s lumbar pain. Spinal fusion surgeries and United States have increased 15 fold over the last 10 years and the population in America has not gone up nearly that much. So it is a fact that with all these surgeries being performed the results have not improved substantially and in fact they remain moderate at best. </p>
<p>4. Only 3 injections of steroids are allowed per year. Doctors specialize in pain management are not in agreement with regards to how many injections of steroids are permitted in any given calendar year. This is because scientifically there really has not been any study that has proved the exact number but should be allowed. </p>
<p>With the steroid being injected into one particular area,  medicine is able to stay in that region and only a tiny bit of it actually seeps into the bloodstream. So truly more than 3 steroid injections can be administered annually but we actually don&#8217;t know the definitive number that is okay. </p>
<p>5. All payment management involves narcotics. This is definitely a myth at this point in time. <p>Injection into a patient&#8217;s facet joint is a procedure where cortisone medication is placed into the tiny thumbnail size joints along the spine that are causing pain. These facet joints are about the size of a thumbnail and they exist bilaterally at every spinal level. </p>
<p>What is the purpose of injection into a facet joint?</p>
<p>Arthritis is one of the biggest problems in this country that generates pain in patients, and the facet joints are not unique. They experience arthritis as people age or after a traumatic situation, and if a person has arthritis at one facet joint they typically will have it in multiple.</p>
<p>Surgery is not a great idea for arthritis and with a facet joint there is no procedure which can replace the facet joint to continue its motion. Therefore, an injection into the joint may provide symptomatic relief the same as having an injection into either her shoulder, knee, or hip.</p>
<p>How long does a facet injection take?</p>
<p>Usually with facet injections the steroid into more than just one joint the patient receives. Each injection may take a few minutes but the sum total of the multiple injections may take around half an hour. </p>
<p>What exactly is being injected with a facet injection?</p>
<p>When a facet injection is done numbing medicine such as lidocaine is typically included along with the cortisone. There are numerous steroids that pain management doctors utilize and usually comes down to what is the doctor&#8217;s personal preference. Options include methylprednisolone, triamcinolone, or Celestone.</p>
<p>Does a facet injection hurt?</p>
<p>A facet injection does involve sticking a needle through the skin. So there is some discomfort from that, however, pain doctors will administer some numbing medicine underneath the skin as well as in the soft tissues leading down to the joints. So it really shouldn&#8217;t hurt tremendously. Patients may also receive IV sedation before and during the procedure which also makes it easier to tolerate.</p>
<p>Is anesthesia necessary for a facet injection?</p>
<p>By and large, patients do not need IV sedation for a facet injection. However, if the patient has issues with anxiety, or even claustrophobia, it may be a good idea to include IV sedation with the procedure. The way it can be done in between simply having local numbing medicine versus IV sedation is to have the patient intake an oral Valium or something of that nature 30 minutes before the procedure.</p>
<p>How&#8217;s the injection performed?</p>
<p>The vast majority of the time people are placed prone for the procedure. Typically a fluoroscopy is used which is a real-time x-ray machine to help ensure needle placement accuracy. </p>
<p>The region over the injection area is sterilized and the patient has vital signs monitor during the procedure, especially if sedation is being utilized.  After the injection, the patient is watched for a time. To make sure the vital signs are stable. The patient is then allowed to go home and if they did receive any type of sedation other than local anesthesia they will need a ride home. </p>
<p>Can patients go back to work the next day?</p>
<p>The vast majority of patients will be able to go back to work the following day after a facet injection. If IV sedation is not used, the patient could actually go back to work the same day if desired.</p>
<p>How long will the pain relief last after a facet injection?</p>
<p>Pain relief with the facet joint injection has variable lasting effect. It may last a few days or may give a few months of pain relief. There really is not a great predictive factor for how long it&#8217;s going to last. </p>
<p>Can a procedure be repeated if the pain relief wears off?</p>
<p>The answer is yes. If the medication wears off after a few months and the pain comes back. Along with this, the patient may benefit from a radiofrequency ablation procedure which can triple the amount of pain relief and that could be well over a year.</p>

<p>Individuals with a tens unit often feel the sensation of pins and needles which is not typically painful but it can irritate the skin. If this occurs, patients should receive hypoallergenic patents. Tens units are typically utilized to treat many different painful conditions such as arthritis, sciatica, back sprains or strains, or pain from scoliosis.</p>
<p>How TENS Work</p>
<p>The tens unit for the patient receives a programming that relieves pain but remains comfortable. The tens unit is programmed to administer small electrical impulses the skin which goes down to nerve fibers. The patient is able to change the settings at will. <p>These 3 varieties of epidural injections work by alleviating swelling and inflammation that the nerve roots are experiencing in the epidural space.</p>
<p>The 1st type of epidural injections that has been utilized for decades is an inter-laminar epidural injection. These used to be performed without x-ray guidance in the office by using simply feel.</p>
<p>Some doctors still perform them without x-ray guidance, however, it has been shown that without it 30% of the time the injection misses the epidural space. So most doctors in this day and age do use fluoroscopic guidance for placement.</p>

<p>This is not an exact science and the hope is that a large amount of medication will make it to the problem areas where there is nerve root compression. Some literature shows that the caudal epidurals can work extremely well for short-term lumbar sciatica.</p>
<p>A new study that was published by the Massachusetts Medical Society looked at treatment of chronic lumbar radiculopathy by caudal epidural steroid injections. This trial was performed in Norway and looked at caudal epidural steroid injections versus saline injections in patients who had over 12 weeks of lumbar radiculopathy.</p>
<p>Patients were followed for a period of one year. At the one year point half of the study participants reported feeling a lot better, and only 27% maintain persistent radiculopathy.</p>
<p>There actually was not a big statistical difference between the steroid group versus the control group. The conclusion was therefore that caudal epidural steroids were not effective for chronic sciatica.</p>
<p>Therefore the more pertinent course of action would be to use one of the other types of epidural injections. This would mean either using the transforaminal or the interlaminar epidurals for chronic sciatica.</p>
